A nonprofit organization serving families battling cancer is seeking a Family Advocate. Qualified candidates have their Masters in Social Work and 3+ years of case management experience.
 
Applicants must live in North Carolina, able to commute to the Raleigh Durham area hospitals and the Durham office at least 2 days a week. 
 
Responsibilities:
  • Complete intakes and needs assessments for families referred
  • Provide individualized assistance, psychosocial support, crisis intervention, and case management to an assigned caseload of CCP families at a level of excellence
  • Coordinate and implement programs and activities to meet the psychosocial needs of families
  • Comply with CCP privacy policies and procedures regarding confidential information
  • Share concerns and suggestions for CCP improvement and contribute to strategies that enhance CCP performance and growth
  • Maintain accurate and complete documentation of family interactions and services provided
  • Contribute to strong relationships with referring pediatric cancer treatment centers that promote timely referrals and coordination of treatment support
  • Advocate and negotiate on behalf of families and patients with local and national systems
 
Qualifications:
  • Master’s degree in Social Work from an accredited university required
  • 3+ years of experience working with children, youth and families dealing with all aspects of care including but not limited to case management, crisis intervention, and mental health.
  • Must demonstrate knowledge and experience in working with multicultural/diverse populations
  • NC driver’s license required
  • Bilingual, a plus
 
Benefits & Compensation:
  • $55,000 – $65,000
